psychiatric problems. In my practice I may sometimes have patients complete one or more of these forms to help provide me
with a fuller picture of their condition(s). However, keep in mind that a survey is only one part of a comprehensive
psychiatric assessment. If you decide to fill them out on your own, I still recommend consulting with
a professional first before drawing conclusions from them.
