Payment and
Insurance:
1.
Payment is rendered at the time of service.
2.
I do not participate with ANY insurance plans. If you found my name in your network, please note that this is NOT
correct.
3. I provide a receipt upon payment,
which patients can submit to their insurance company for out-of-network reimbursement. However, patients must check
that their plan includes out-of-network benefits first in order to
guarantee reimbursement.
I also do not accept credit or debit cards
Appointments
and Cancellations:
Appointments may
be made by phone or email. I do have a policy of charging for missed appointments or appointments cancelled within less
than 24 hours. Exceptions may be made in the event of a true emergency on a case-by-case basis. Also, if I can fit you
into an otherwise unfilled spot within a week's time, I will usually not charge for the missed session.
Privacy:
All doctor-patient communications are kept confidential unless I feel that to do
so would compromise the safety of my patients or others. Private patient information will only be exchanged with third
parties with a patient's written consent.
Office Hours:
I have flexible hours during the week,
with select weekends and evenings (usually Tuesday or Wednesday) also available. I am generally in my office Tuesdays,
Wednesdays and Fridays after 3pm, and Thursdays from 9am-5pm. However, I am not in the office on Mondays.
